The hard copy AOR mentions use of representatives (IMM 5476). It looks recommendation than mandatory.

In the application package, I did not send IMM5476.

My mother gave the medical on July 19th in Toronto but the medical office here is very slow in submitting. Avoid https://www.panelphysiciantoronto.ca if you don't want to delay the medical process

I think we have had 2 cases in this forum with above mentioned physician.. have you guys thought of writing reviews for them on google so that it helps other people?
