Not every form in the checklist is mandatory for every person. If you don't need the extra space, this one isn't for you. If the box becomes grey, they don't want that information in this case.

Thanks usr19 for the quick response. Do you have answer on my first question whether my grandmother would be a primary applicant as well? If yes, is that a separate application for her? or does it need to go in the same package with my mother's application and my sponsorship docs.

No. Maternity EI is just insurance money which is paid because you have paid insurance premium on your pay. EI is taxable income.

The first item on the Quebec checklist is the letter from IRCC inviting you to apply for the CSQ; that said, explain to me, how are you going to attach this letter if you haven't even applied with the federal government ?

Just answer you haven't applied for CSQ and leave the rest blank, no problem with that

your sister is not consider as "family member" in this application. Only parents and their dependents are consider for this question.
