Details of Military Service form

Hi folks,

I got a request from CIC regarding Details of Military Service form. They enclosed Military Service form to their message.
Should I just fill the form in ??
Am I supossed to provide any supporting document?
And some info CIC asks for is considered as a military secret in my country. Is it ok if I left these spaces blank?
Can anyone with this experience help me ?

CIC put a deadline till 6th of October and I dont know what to do.
