Schedule A – Background/Declaration [IMM 5669]

" Application to Sponsor a Member of the Family Class you submitted to this office on behalf of XYZ

We are pleased to inform that you have met the requirements for eligibility as a sponsor

A request has or will be sent directly to the principal applicant with instructions outlining the
requirements for submission of the Schedule A to CPC-Mississauga. If your principal applicant or their
representative has not yet received this request, or an invitation to link their application to My CIC, it
will be sent in the next 5 business days. "

I received this email from CIC on 8th June ,but my wife has not received the IMM 5669 form either in email or CIC account where she had already linked her application till now its been more than 12 days ,and they have told to email the form with in 30 days.
she has received her medical examination form on same date as i got the above email.
How do i go about this kindly advice.

The same thing happened to us and we called and they said they were waiting for it even though we never got instructions, so we just mailed it.

Hi, we didn't receive this as well on the end of the applicant and was just planning to mail it. Did your applicant physically sign IMM5669? I was hoping to mail the actual form as we're not linked online to myCIC.

We don't either (we have lawyers). He physically signed it. We priority mailed it to Required Documents CPC Mississauga/CTD Mississauga PO Box 6100, Station A Mississauga L5A4H4

We have a representative as well which is why I didn't link it. I was just afraid that our 30 days would have lapsed without us knowing about it.

Ya no one seems to get those additional instructions. What country are you applying from?

We're doing an Outland application. My PA is in the Philippines. How about you?

Looks like that would be the plan. Print off IMM5669 for my PA to sign then send via courier to Canada.

You should probably do the following:-

1. Check your MyCIC account, if there is an option showing up to upload Schedule A, do it.

2. If it doesnt show you can print a copy of the filled Schedule A, sign it and mail to the following address

CPC Mississauga
P.O. Box 6100, Station A
Mississauga, ON
L5A 4H4

3. Alternatively, on a safer side, along with mailing, you can upload the soft copy on IRCC Webforms too.

By the way, anyone here who just submitted the IMM5669 without the PCC?

Since the principal applicant didn't receive any further instructions about this except for me (sponsorship approval), there was nothing about the PCC.

Our application is through a representative and he says not to include the PCC because it doesn't state in the letter. I'm just confused how to go about with this? Do I or do I not include the applicant's PCC?
