Hi, I have been with the same employer ( 5 and a half years ) since I got placed after my MBA.
I have held two different positions with them.

Now, in the form,do I just mention the current designation ( and add the documents confirming appraisals etc for the 2nd stage) or should I specifially mention the same employer twice with the respective designations/dates on the form ?

I would assume that since your designation changed your role and job duties also changed . In that case find NOC codes for both the designations and show them sepratley
