Where to get the immigration medical examination report form (IMM 1017E)?

Hi,

I need to get the immigration medical examination report form (IMM 1017E) before going to the panel physician to get my medical exam done. I wonder where I can get that form?

If I’m not mistaken, the IMM 1017E should be sent together with the letter of request for medical exam with the name of the client.
