i have read that one of the requirements in applying for proof of citizenship (for minors)
are medical records of monthly doctor's appointment (for newborn applicants) since im in a conjugal relationship with the canadian father of my child,
and we are not married here in the philippines, should i use my last name or his? please note that according to the new philippine family code the baby will be able to use its father's last name provided the father will acknowledge the baby by signing at the back of the baby's birth certificate.
we dont want to have much problem in the future when we apply for the baby's canadian passport and proof of citizenship. please advise me what last name to use on the medical records. thanks
