I have only one problem, that I am self employed family physician. what should I do about letter of reference from the employer ????????

First of all start by writing your CV/RESUME and put in details what your job functions have been (in detail).
As a Self Employed Doctor you should and will need to prove what you have been doing and also that you are and have been active.
Medical Registration Certificate, Tax Return Certificates, Invoices to your clients, Business Bank accounts transactions, in fact anything that can prove what you have been doing .
As a Self employed the Onus is on you to prove your responsibilities.
