Visa office & where I live aren't the same country, where to take medical exam?

Hi all,
If my visa office is Buffalo, but I am living in another country at the time I have to take medical exam, will CIC refer me to the doctor in the country I am living OR do I have to go to Buffalo?

a quick note, i am not sure how they let you know who the doctors are. Perhaps they live a list of doctors only in your visa office country or perhaps it is a website. Perhaps someone could tell you about it on here.
