verification of employment and reference form???

hi! i am trying to have my credentials assessed to know if i still lack requirements to apply as a RN in ontario. one of the requirements is to show proof of recent safe nursing practice, which is through a reference from my employer. they're saying that i must ask my employer to complete the verification of employment and reference form. But the thing is, this form is not in the package they sent me. Can i just send my employers references together with my application form? thank you!
