Use of a representative

I got my application package back today saying "Form IMM 5476 in your submission indicates your representative is not an authorized paid immigration representative".
I am not using any representative and sent the "Form IMM 5476 - Use of a representative" blank. Should I just send the package back without the blank form? Please give your suggestions what I should do.

Read the instructions! If you don't have a representative, you don't send the form.
