Thanks rockstar , it does give me hope - I did exactly the same although the checklist asked for the form to be submitted by each member.

I am in deeper waters - Relied on DHL to courier the docs and they have lost my package, entire day I was following up and they say it just disappeared & will keep me informed - Any suggestions/opinions will be helpful - sorry I am completely at loss for words as they say this is a one-off incident but for me, this is an oppurtunity that is lost.
