I have got medical forms from CHC delhi. I lately realised that there is a mistake in my wife's name. It is not as per the passport we submitted. It has got suffix , which is not as per passport and we do not want it that way.
I am left out with merely 10 days to my deadline for finishing my medicals.
my query is
1) Can a DMP make a change in name ?? (It is not a spelling mistake, it has suffix and we want to remove it, as per passport)
2) Or we have to ask for new forms from CHC delhi ? , if yes ....then will they extend my 45 days deadline ??
how do i handle above situation ???

1.) NO DMP can not change the name and even if he does that does not change it in COPR that you get from VISA office
2.) Write to VISA office explaining your situation and seek guidance from them. They may ask you to send new forms with modified name and may add some delay.

: Send a mail to CHC ND telling the mistake in medical forms. They will send you reply within 24 hours and the mistake would be corrected in record. Give that reply to the DMP with medicals. The medical forms then can be used.

As Ankit said, inform CHC about the error in the forms and they will issue you new medicals forms soon. It has happend with many applicants who have shared such experience.
I had problem with my name on medical forms , doctor corrected it and said to send covering letter with residence fee and copy of passport and ask CHC to correct name on record.

I talked with DMP , while taking an appointment....he said...i will do the change and i am authorised to do so.

gng2canada, please check with romford, if his name was incorrect in medical forms or did he gave incorrect name on all forms etc for Visa purpose.
If your application has correct name and Medical form has incorrect name that is another issue and the name in application forms itself is incorrect is a different issue
