I've done my upfront medical exam for study permit on 24. Dec 2016
and Since my CRS score is around 472 I'll apply for Express entry soon around June or July
Can I apply with that old medical exam as it's valid for one year , or do I have to call my Physician to change the status from study to permanent residence or do I have to submit a new one ?
My status: I'm outside Canada at the moment and I will be international student in Canada On June or July 2017.
Why dont you apply for your PR first? Your tuition fees will be waaaay lower.

no such big difference in the program I've chosen plus It wasn't in my plan

I'm planning on getting into ph.D once I land as PR.
Would it be easier? admission wise? tuition fee wise? permission to work as many hours as I can?

I'd say it's better for you to apply for permanent residence first Entry express as you will be treated same as Canadians
Yes much easier you will be able to work whenever you are free unlike international students only 20 hours per week
