i will just need to bring my passport, picture and ITA right? no form needed?
then the result will just be uploaded directly to my CIC account?

I would say the safest would be find your local panel physician with IRCC website (http://www.cic.gc.ca/pp-md/pp-list.aspx)
give them a call and ask them what do you need to bring with you for the appointment.
I dont know about other countries' panel physicians buy in Canada they will just input it in a data base where IRCC has access to.
Do you guys know how much they usually ask for the doctor appointment

Since you are doing an upfront medical, you don't need the IMM 1017E.
By the end of your medical exam, the attending nurse will report to you the results of their findings...if everything is ok, they will be sending the results to CIC
