Hi all,

Please help me out with this issue. I am unable to save the medical request form. I am selecting "Yes, I want to complete a medical exam" and after that I am saving the document but unable to see my selection when I reopen it.
 
Ok done. I used foxit reader and selected the desired option. It is working with foxit reader.
