Only last question- the country specific guideline says that we need 4 photos. When he went to get the medicals done, they printed 6 pictures- 4 for the medical form, and 2 extra that they gave us in an envelop. Since we only have 2 photos not attached to any sort of form, do we need to go get 2 more? Or is the 4 required photos referring to the 4 needed for the medical paperwork?

You need photos for the application, photos for the DMP, and photos for the COPR. The exact number varies (my DMP wanted six as I recall) but I think it was 4 for the original application, two for the COPR and then whatever the DMP wants. Also, be careful to watch the specifications, since each use seems to want a different size.

Interesting. According to the IMM 3910 that I used, one of the requirements was "four immigration photos":

Supply four (4) recent photos for each member of your family and yourself. Follow the instructions in your guide (see Photos in section on completing the Application for Permanent Residence in Canada) and in Appendix B: Photo Specifications.
And Appendix B was a complete specification for immigration photos. My recollection was that the DMP wanted passport sized photos - I got them at the same time and I ordered different sizes. Six passport photos, four immigration photos. When the PPR letter arrived, it asked for two immigration photos.
In my GCMS notes they specifically noted "bio data not on photos" - I remember seeing that and going back over the directions, but the US directions don't ask you to write anything on the back.
Two for the PPR. My understanding is that one will be attached to the COPR that is sent to Sydney for them to issue your PR card. I'm not sure what happens with the other (I think it's supposed to go on your COPR). CIC seems to love asking for pictures they don't need (they ask for work permits, but don't use them in my experience, for example.)

The problem with English names are that they show up over and over again. Ever heard of Boston? I've been to the original city in England (and laughed at "Pilgrim Memorial Hospital").
