Dear Fellows,
We got Medical call papers today, In Nov 2010 we sent our papers with all documents. people we need advice once again from your end.
1- New schedule 1 background / declaration form for yourself and each family member over the age of 18.
{applicant & spouse. Question 11 : please comlepte personal history since the age of 18 (no gaps)in the correct format. a writen statment cannot be accepted} this is writen with ball point.... (the above mention requirment we already sent them in Nov 2010.they asked again)...what should we do.

2- new additional family information form IMM5406 (Only Spouse)
3- Right of permanent residence fee

i have ask 3 questions up there
1st question about "schedule 1 background / declaration" are we need to fill whole form or just question # 11 only??? we already sent them this form in NOV 2010.was that wrongly filled?? we attached extra papers on which detail was writen. guide us in this regard.
2nd question about spouse additional family details form IMM5406
3rd one regarding fee of PR ...only principle applicant gona pay this 490 $ or spouse have to pay also 490 $

Oh did u apply for yourself first then added ur spouse after you had applied?

I'm not sure about the other questions but for the fee both u and your spouse have to pay it.

No...we applied together and the information they required above we already gave them but i think they didnt want that information in a way i gave them,i want to know after 18 years of age details we cant write everything on that form,we have to attached extra sheets to complete that information. any one who write to them and who??/

AOA,

1: Just fill in all forms for schedule1/declaration, add additional sheets with proper format or you would just write (Please see Personal History Addendum and write different for additional applicant ) in Q11 Personal History and write everything on new sheets in proper format.
They usually ask this to get updated if there are any kind of personal history changes of applicant. I submitted it myself.

2: Fill 5406 again and send them for spouse

3. Fee will be for both of you.

you are so lucky, i submitted my complete documents in 4 Oct 2010 and till to date i didn't receive anything from them. its almost more then 6 and half month passed. well best of luck to you for the rest of your case. i apply in SNIP. what's your PNP????? you get medical request through mail or email..if through mail then which courier service they used to sent medical request.
