My mother in law is currently visiting. She is on an extended status visitors visa until Dec 2019. I would like to apply for a Supervisa for her, however she is 90 and no one will give her medical cover. Is this a problem? She I just apply without the medical?

The short answer is no. If she applies without having medical insurance then she will be issued a regular TRV. To qualify for a super visa, she must have medical insurance coverage.

Scylla can answer as well and this is only my view maybe the latter just extend stay although if were me I would still have a concern about no medical insurance for any emergency requiring hospitalisation which at 90 plus odds can be high but I guess you are prepared for that. How many times IRCC might extend would be an unknown factor regardless same as no guarantees with a new TRV given living in Canada versus visiting.
I would also be concerned about my financial liability in the situation. Can you afford a very large hospital bill? What happens if your MIL gets sick and is not able to return to her home country can you afford her medical care for many years. I'm not trying to be cold hearted but it could end up costing hundreds of thousands of dollars. In other cases maybe only thens of thousands of dollars. Funeral alone is easily +10K.
