Can anyone tell me, we have to fill only 5 forms for study permit given below:

1) Application form
2) Family information form.
3) Student Questionarie form.
4) Use of Representative.
5) VFS consent form.

Any other form other from this. Reply ASAP.

Yes, these are the five documents/forms to be filled. Out of which 4th one would be filled by your consultant.

If you are applying thru VFS,u will be advised by them not to fill the representative form.I guess u have got all the forms covered.

I think you want to say, "If you are applying thru VFS without help of any consultant, u will be advised by them not to fill the representative form."

Representative form is not of any concern to VFS. But if one does not take help of a consultant or does not want to display he/she took help of a consultant than yes that form is not required.

I have downloaded these five forms from CIC website just to fill them roghly b4 filling original forms. I am applying through canam consultant mumbai, so 4th form is neccessary to fill na? And VFS form also becoz' my file will be sent by VFS from mum to delhi.

So i will have to attach both 4th and 5th form with my file. Am i correct?
