Study Permit amendment concern please help asap!

My study permit has no condition that either allows or disallows me to work on or off campus. My question is, do I need to send my birth certificate along with the rest of the documents?! I only need a condition printed; I do not need my name adjusted or any personal information fixed

Yea pal. I also downloaded the form, filled it up and printed it. I just want to know if my birth certificate is needed or not
