I`m applying for PR via common-law sponsoring. I`d also like to apply for an open work permit along with my PR application. I downloaded the documents for the open work permit, and one of the documents is the IMM5409 ("Statuary declaration of common-law union"). Do I need to fill this form out / go to a notary to have it signed? If so, should I then also mention this document in my PR application? In the main application I didn`t read anything about that form; it was only brought up afte downloading the work permit docs.
