Spouse name

Hello,
My name is saurabh teckwani. My wife name is jasmine sharma.
in my passport, my spouse name is printed as Jasmin saurabh teckwani. However her real name is Jasmin Sharma.
how should I fill my ita form.
Should I choose YES in my wife for for “ have you been known by any other name” ? And in the options for type of name, should I choose alias or OTHER name ?
please help
