Yes, it is the same number. In general you should only have one per lifetime (I ended up with two, but there are even CIC procedures for reconciling the two and I have seen signs that someone has done this.)
I'm filling out the pdf forms on my computer. I read somewhere that you can't leave fields blank, but you have to fill in at least N/A. In the questionnaire is for example a section about common law/conjugal. This section (Q21-23) doesn't apply to me, so I try to fill in N/A in these fields. However, I can't fill anything in these fields. So do you leave in blank, or do you write it on after printing the form?

No, only if u have children. If you wish you can print it off and write a big NOT APPLICABLE across it to be sure they don't think you forgot the form (which they most likely won't think that, but to be on the safe side...).
