There is a contradiction in place of birth between your passport (Wah) and the place of birth you have mentioned in the forms (Sargodha), so this might be causing some confusion. I am sorry but I have no suggestions for you at this point.

No.. in passport and and in forms I have mentioned sargodha... its just that in B-form Its written wah .. now i dnt know whether they had considered reviewing my B-form in presence of NIC...

its just B-form which was considered as birth certificate... There was nothing as birth certificate which i sent, neither they demanded again

It is not mentioned in the checklist. It only appears in the country-specific requirements documents that's why I am so confused.

I remember only PA has to fill it.

Yes those are for the PA only. You may be a Pakistani national but you aren't applying for your immigration. You are a Canadian resident applying for your spouse's immigration.
