Hi guys,

Stupid question, as I am the husband, who is sponsoring my wife to become a PR in Canada, do I have to fill out a separate Schedule A Background Declaration Form (IMM5669), and Additional Family Information Form (IMM5406)?

No, only the applicant(your wife) fills out IMM 5669 and IMM 5406. She has to put your name on line 2 of IMM 5406 under spouse/common-law partner.
