Sponsorship Evaluation, Use of representative, & Spouse/Common-Law Questionnaire

I am working in Canada on ICT (Intra-Company Transfer) for a period over than a year. My spouse accompanied with me since my arrival here. My spouse landed here as dependent however applied and got open work permit thereafter till our visa expiry. Now, I want to apply for PR under CEC category myself i.e. without any help from Lawyer or anyone else. Do I need to fill following forms:

1. IMM5481 (04-2012) E {Sponsorship Evaluation}
2. IMM5476E (03-2014) E {Use of a representative}
3. IMM5285E (08-2013) E {Spouse/Common-Law Questionnaire}
