Is this not the same as Document Checklist [IMM 5491] (PDF, 159 KB) January?

I know one asks for photos and one does not = is the checklist IMM 5562 (05-2004) E a current form or redundant?

are you being sponsored under family class or using another program? The only place I see form IMM 5562 being used is in the PNP stream.

If the checklist doesn't list it you don't need it. I never sent it in the first time and they didn't ask for it. If you follow the checklist and sign all the needed documents, you're all set. The list you posted is THE list.
