Once they accept the sponsor to sponsor their spouse, when the application is sent to the visa office, does the visa officer look at the sponsors forms again or just the applicants? And if so, how do you know for sure? (do they mention it somewhere on the site?)
The sponsor's eligibility must remain in force for the entire duration of the application, from submission to CPC-M to the time of the applicant's landing in Canada, so yes the VO will again assess the sponsor. It is not explicitly stated on the site, this is inferred from the legislation itself. Read Sections 130-133 of the IRPR regarding sponsors.

The visa officer most definitely looks at the sponsorship documents - it is the deciding officers responsibility to assess the full application.
It is stated explicitly in
IP-2 (in defining roles and responsibilites) that CPC-M makes "recommendations" (bolding on original document) on spousal sponsorship, whereas the Visa Offices "make decisions on spousal sponsorships."

Yes - it happens. Ultimately, the VO officer is on the hook for who they approve so they will check the application meets the requirements at the time the visa is issued.
There are examples people have shared on the forum and you can see others on CanLii where applications are rejected during step 2 for reasons pertaining to the sponsor.
