Re : Country Specific instructions (Hong Kong) -IMM3903E (01-2016)
Item 9 - Identity and Civil Status Documents : all the doc provide by copy is fine ? and or needs anyone to sign for certify true copy ?
if so, pls advise who can do so ?
Item 13 - Police Certificates and Clearances : apply before submit the form and or receive further request after CIC review all the document ?

Item 14 - Proof of Medical Examination : apply before submit the form and or receive further request after CIC review all the document ?

Item 15 - Photos - as per requirement, the back of photos, understand needs photographer provide the chop with their complete address , name and date.
how about the name and date of birth - needs any specific people to sign on ? and or just handwrite by applicant ?

Appreciate your prompt reply and help

Documents which are in English only need photocopy. No need to certify them to be true copies.

HK police report, you will not be able to apply for that upfront. Just put in a note on the check-list to state this...never hurts to remind CIC that you are not missing any documents.

Medical: you send in the form given to you by the doctor. To be safe, you should always keep a photocopy. CIC will match up the medical to the application. Some people wait until SA or AOR2 to do the medical, which will possibly mean a later COPR expiry date, as expiry date is same as expiry of medical which is 12 months. On the other hand, would it make the application slower?--I am not sure if anyone can be 100% sure, so it's up to you. I know of people in HK who do medical when reminded by CIC instead of upfront in order to have a later COPR expiry date.

Photos: follow the instructions regarding what needs to be put on the back...name and address of photographer, date of photo taken, etc. If photo studio does not have a chop, he can fill in the details by hand writing. If he will not fill in the applicant's name and DOB, just do it yourself.

For IMM5669, fill in back to 18 years old instead of just last 10 years (unless the applicant is younger than 18 of course)...high chance that HKVO will ask you to re-do it.

You may also want to fill in "use of representative" to appoint sponsor as the representative. It makes things easier if the sponsor will help make inquiries about the case. Even the spouse has no right to access the applicant's information, unless with written signed consent.

Correction: the HK guide now says medical instructions will be sent after receiving the application, instead of upfront medical check
