Can I write answer to more than one question of the same from in one Extra sheet of Paper or I should use one extra sheet for each question of the same form say Schedule 1?
I am providing a format , is ok? please , provide me information how to write additional sheet in details. In the appointed table in the form the space of on cell is inadequate, so I wrote N/A there and need to answer in the separate sheet.

There is no standard form of the attachments but it should be very clear, concise and logic. Your example format is ok.( my opinion). i don't know what other forum members think.
