We have applied as self employed physiotherapist. For this we have to provided the 3rd party evidence in our case we have provided the reference letter from the patients along with the payment slips. In addition we have also provided the reference letter from the hospitals who usually refer their patients to us. Moreover we have provided the rent agreements, shop establishment certificate, professional tax receipts, clinic snaps, visiting card.
