Second Time Visitor Visa from Washington

Hi there,

I am going to Canadian Consulate in Washington DC for visitor visa. My previous visa expired 1 yr back. I have few questions:

1. Do i need any appointment before going to consulate to obtain visa?
2. How much is the fees for Multiple Entries visa? at that time it was $150?
3. How long they take to return the passport with visa. Do they return on same day or next day?
4. Below are document list which i am taking with me:
Original 797, Employment Verification Letter, Passport, Bank Statement, IMM 5257 A and IMM 5257 B , Passport size photos.
Is there any other document i need to take with me?
