Schedule A

Hi,

I am trying to fill out schedule A for spousal sponsorship application. I have police certificate and I want to send my police certificate with schedule A. I want to send my application package along with the police certificate and schedule A.

Can somebody please tell me which schedule A I should fill out? There are apparently two schedule A's

1) Schedule A IMM 5669 (12-2012) E
2) Schedule A IMM 5669 (06-2017) E

Which one is it that I should send with my application package? Can someone help please?

I know I can send it online but I want to send it with my package.

The Schedule A IMM 5669 (06-2017) E is the updated version as of June(06)/2017. Send the updated form.
