Sample of reference letter needed - Urgent

Hi All,

Sincere apologies for the new post. I urgently needed a sample of reference letter which any one of you has used to get employment details from current/previous employer.

I remember seeing a complete sample somewhere inside some thread and have been searching for it since 1 hour but in vain.

Please help. Thanks.
