Requested form IMM5475 "Authority to Release Personal Information to a

I got the AOR on January 31st, yesterday I got a message from CIC : Requested form IMM5475 "Authority to Release Personal Information to a Designated Individual"

Does anybody know? why? May I designate to my wife? or leave it blank?

By the way the form ask me for this: Office where the application was submitted and LOcation of Office. In my case I am in Canada, CEC stream. All was submitted online. What can I write there?

probably your visa agent has submitted the form and they need to be named as representative. not sure.

Hi, I have also been requested to submit the same form while I did not choose any representative during filing.Did you guys figure out why the request was made?What you guys submitted?
