Request of Academic Records- IQAS-Urgent

Hi Friends,

I am planning to get my ECA with IQAS next week, Just wanted to know whether Request of Academic records form is necessary? To send along with the package? Since IQAS accepts the transcripts only from the University I am not sure University people will attach this form along with the transcripts? anybody who has sent without Academic records from and got their documents evaluated?

I also have the same issue. If you were able to find, can you please let me know if IQAS excepts the package without academic-records-request-form
