Request for additional Family info IMM5406

Hello everyone,

I applied for Express entry under CEC on May 15. I am the principle applicant and there are no dependants. Today I got a update requesting for additional documents. The document requested is IMM5406 Additional Family Info.
Any reason why they would require this?

The requirement states : A separate and newly completed Additional Family Information form (IMM5406) bearing original signatures.
I assume they mean my signature, correct?

i have submitted imm5406 for 10 days now but still haven’t heard anything yet

Yes, I already recieved my PR on July 31. There was no update to my profile after I submitted the form. I directly received PR.

you mean you already receive your PR card ? Or confirmation of PR ?
oh when did you submit the imm5406?
you probably waited for more than a month to hear back from them ?

I got same request letter today for My PR application. I am the principal applicant and there are no dependents in my application.

In the form there is check box
Appointing representative complete A and D
Cancelling the Appointment of representative complete A, C and D and

For me neither I am not Appointing any nor I didn't Appointed anyone to cancel. Also, I am not sure about Name of the office where Application has submitted.

Not sure how to fill this form now. Can anyone please help.

you need to download imm5406e form and fill out details and attach to your application. imm5406e is correct document

Thank you for the quick reply. It turn out to be a special document which should be opened with Adobe. I managed to open and fill out the form. However I am stuck in the last part. Signature... Should I sign it electronically using Adobe signing tools or should I just write my name in the space. I added the photo of that part:

After signing I will save and upload the pdf file to my online application i suppose

In case it helps, I was requested the same doc. I filled it in with Acrobat, then printed as PDF all pages but the ones with the signatures (d1). Printed the signature page, signed, and scanned it (d2). Then, I merged both pdf files d1 and2, that I submitted. It was accepted.
