they will ask for extra document in which step?
when on line status recived by visa office? or in process?

Documents can be demanded at any stage till visa stamped..off course u will remain in process as long as Decision not made

Very rare chances.
they asked me to give my Bachelors and Masters Certs and Transcripts once again. This surprised me. I am sure they were not surprised as me in asking for the same thing that I provided them before.

This is in response to your PM...
The e-cas status doesn't matter, 'coz it's not updated regularly as the case progresses. I have many a times suggested that we shouldn't depend wholly on the e-cas, it's just to see a generalized progress.
As with the ADR (Addnl Docs Request), it could be asked anytime during the case processing till it's conclusive decision, ie. till u have your PRV.
ADR could be of very many types, repeat forms, repeat Certs/Exp Letters, updated POF, CIC Questionnaire etc. Anything & everything that may require the VO to assess or reAssess the file, or for just a repeat doc to Xcheck consistency of previous replies.
Rather than the e-cas it'd be wise to know/observe one's timeline... Generally speaking, one should expect the 1st ADR somewhere around 2-3 months from the 2nd AOR.
And also we should remember that one ADR might not hold value... there could be many more to come... Till the DM... Two, Three, Four - any nos.

Dear Qorax,
In my case i was not ask additional documents and f or fear that they should not reject me i decided to se nd some additional documents.
My reasons were that my employers details had changed like phone numbers , institute also changed from College to University and the Dean was also changed. For this reason i asked for new recommendations letters and sent to the visa officer. I sent the documents when my file was in process and it is still in process, since june 29 i am still waiting for next update.
I hope it should not be a problem that i sent documents they did not requested.

PR Golden Rule:
Send only what has been asked for - nothing more - nothing less - not 1 doc here or there.
But, with those docs that u sent again it should be alright... Neither a benefit nor an adversity.
