Remove conditions on work permit

I am applying for one or more of the following:
1. a work permit with the same employer
2. an initial work permit or a work permit with a new employer
3. restoration of your status as a worker
4. temporary resident permit

My situation is the following: I have a work permit with restrictive conditions, due to lack of medical exam. Now I have the results of investigations and I apply to remove the conditions on my work permit.
I don't know what to choose from the situations in the above question. I did not find a job yet.

No. I came to Canada under family sponsorship.

I applied in February for permanent residency, so in January, next year, I expect all formalities to be done.
