sorry to hear this...
when u uploaded the pcc in upload field did the upload field show the message stating "provided" also did you tell them that if i had a pcc which is dated before the AOR why would i keep it with myself?
i explained that i had pcc before i applied and all other possible reasons. i am just waiting for 30 day period to end so i can get a reply from them for CSE
You probably have a good case to have it re-opened. Hope you also attached the screen shots as evidence? You may also want to create a new profile, just as back up for any eventuality.
Application cancelled on 15 dec due to missing PCC(Error)
Put through CSE on 15 dec.
Got a mail from cic stating - We are forwarding your information to the responsible or concerned department, they will contact you if they need further information. - january 4th
What does this above statement mean. Is this a good sign or very normal to get this replyÉ

It means that they are trying to connect the information in your CSE to your EE application. If this goes through successfully, your application should be re-opened (unless there is another issue for you to address). summary: Good sign.

In my Express Entry profile, My Background Check was at "In Progress". I received an email on Saturday that profile has been updated and when I check it is changed to "Not Needed at this time". The last changes was made on 20th December, 2015.
My Medical is Passed and Eligibility is "Review in Progress" state.

Need help
I got ITA on dec 4 and submitted my application yesterday. Today I noticed that I have not submitted my bank statement for my investment account for 6 month. (investment accounts usually don't have balance statement and bank didn't give me any). It is not significant amount and I have my fund in my saving for which I have a complete bank statement report.
and for my ECA, I have a bachelor and masters. ECA evaluated them both in one file saying :
ECA summary:masters,
credential 1-equivalent to bachelor,
credential 2- equivalent to masters,
should I have evaluated my degrees separately?
I am really worried. are these serious mistakes? what can I do?

As far as your savings statements shows you have enough fund, you don't need to submit anything.
ECA is fine. I did the same thing and no issues faced.

I worked with XXX company from 2011 to 2012 and got experience letter from the company which doesn't mention, that I was full time and my salary and now company is closed and not in operations anymore.
I used the same experience letter when I applied for my study permit in 2012, so, must have this letter.
