quick question about format

Is it advisable to include a post-it note when I need to add a little something, (ie. explaining that my mailing address will be in Canada in a months time (i'm moving home to start a new job) and that my current residential address is in Europe).

It's OK, but post-it notes can easily come undone, fall off, and get misplaced. I'd just write a note on A4 paper and attach it to the forms.
