I have a small question regarding the "Use of Representative" Form No. IMM5476. I am applying for the Canadian FSWP on my own without any help of any representative. My question is, do I still have to fill up the form and submit it with the application? Any inputs/suggestions are appreciated.

Since you are applying on your own, there is no need to fill up the representative form.
