My application was received at CIO in Sydney on Dec. 7 ,2009. I got the letter from CIO Sydney office tofday. It said I have failed to meet the application requirement.
- You have failed to submit the required processing fee or the fee you submitted is not an acceptable for of payment (see appendix A) and or fee payment form (IMM5620)
- The individual indicated on the use of a representative form(IMM5476) which you submitted is not an authorized representative (see appendix B)
The first one. All I did is fill up the form IMM5620 and said I want to use the credit card payment. I put all the info for the credit card. So this one I will fix it by use Bank draft.
The second one I did not submit form IMM5476 since I did not use any representative. In IMM5612 form said I have to submit if I use one. Did I do anything worng.
If you have filled IMM5620 correctly including date and all relevant CC details, it is surprising why they responded like that. But Moving forward if you decide to resent may be draft is a safer option.
It is even more surprising to see second comment. You are right that if you do not have a representative you are not supposed to fill in IMM5476
That is what i intend to do this time. I'll mail bank draft.
For the "Use any representative" form, I did not put a mark on the IMM5612. I did not attach IMM5476 with the document. I'll try resubmit again with out the mark on IMM5612 and no IMM5476 and see what happen.

we have just received a letter from sydney ns, returning our application.
we have the same problem with "use of representative form" indicating we submitted is not an authorized representative. but we didnt even submit and checked IMM5476 on the document list.
we dont get any representative, we apply directly.
