I need some clarity to the question since it does not say "in the past 5 years....".
Does this question only covers the 5 year eligible period only? The reason I am asking is my wife had US TD temporary visa and stayed for 4 years prior to coming to Canada. This US visa was outside the 5 year eligible period. Does this mean NO?
Is the question basically asking if anyone has permanent status like Green Card or whether one has more than one citizenship?

From my understanding this is not restricted to the last five years. It says "Have you ever...". So that really means all time. So I would say your wife should list the US visa.
On the old form they only asked for citizenships and for PR/Green Card and equivalent. It seems now they want to know everything, unfortunately. Why I don't really know, but I think "Have you ever..." is unambiguous.
I wanted to click "no" but then got a pop up saying "even if you got a citizenship from the country you were born, you have to click yes"
So I selected "yes" and entered "Belgium, Citizen, -my date of birth-"
I have no idea if I did the right but I though to myself, better safe than sorry.

I want to point out that, in French, the meaning is TOTALLY diffrent. In French (the language I filled it in) is: "Avez-vous déjà obtenu un statut d'immigrant ou la citoyenneté dans un pays autre que le canada ou que votre pays de naissance"

I would not mention tourist visas. Only those that allowed you to live more than the country's visitor stay limit. For example, my wife had a TD Visa which allowed her to stay in US more than 6 month visitor stay limit.
