Inland applicants get their CSQs faster than outlands with Quebec's PEQ program.
They are already prioritized.
(For ex. I applied for CSQ in Oct 2014 and got it in Nov 2018 - It took 49 months for me.)

Also, all additional points such as integration, French language, Quebec educational history, etc. give extra points to inland applicants in the CSQ stage. This is already a huge advantage.

But...
In federal processing, there are no checks for integration concerns, language, or education because these are already done in the CSQ phase by MIDI. PR processing in IRCC is normally straightforward and equal for all applicants regardless of their physical location.

So; the application should be processed in FIFO format or chronological order.
It is totally normal that an applicant who is applied 1 year before others should get his DM first.

And for we are paying our "taxes" argument;
People are paying taxes and they are getting benefits of all municipal and governmental services while they are living in Canada.
This is the cost of living and something people benefit from hospitals to post offices, subway lines to thrash collection.
Paying taxes does not make someone privileged at all, it is something everybody has to do.
Then you should have come to Canada as a temporary resident first like all inland applicants to benefit from all of the "benefits" you mentioned. Inland applicants are directly contributing to the Canadian economy by working here. I hope you realise that maintaining a legal status and ensuring that we are legally allowed to work while waiting for PR is a stressful phase. It's not because we're already here that they serve us work permits or any type of legal status on a silver platter. Outland applicants; once they land, they don't have to wait a lot before they become eligible for the "benefits". They don't need to worry about another permit being expired! Borders were closed and travelling is not encouraged right now. It makes more sense to give priority to inland applicants!

Ramq confirmed to him that under implicit status he can renew his insurance. He escalated the matter and a supervisor replied that he's eligible.
My point is that you are not considered as having an "Implicit status" if your work permit has expired and you are waiting for a new while your PR application is submitted and/or in process.
You know how when you are applying for a PGWP, you are allowed to keep working until a decision is made? This is having implicit status. In the case where PR file is submitted and/or is in process and applicant is waiting for a new employer specific work permit or for a renewed work permit, he/she cannot work while waiting for a decision about the work permit. He/she does not benefit from "implicit status". :)

Yes it was surprise to me also . I randomly check ECAS and was surprised to see Decision made.
Can anyone please advice what's next step after DECISION MADE status in ECAS ?
Just was in the same situation (but outland), in around 1-2 weeks i got invitation to send passports for visa and COPR.

In Québec, we don't benefit from "implicit status" when applying for a work permit while waiting for PR! If he does not have a valid work permit at the moment and if his visa, too is expired, until he gets his new work permit, he is deemed to be without status. This can explain why his RAMQ isn't getting renewed.
That is not true. Since Spring 2020, we are able to renew the RAMQ card, simply sending them a proof that we submitted the work permit renewal application before its expirity. It would be horrible otherwise, as the waiting time for the work permit renewal is now 6 months. It would be 6 months of paying taxes and not being covered by RAMQ!
And we do benefit from Implicit status in Quebec. We can work while waiting for a renewed permit. I have been there once and doing that again for the second time...

I had to renew my passport as it was expiring and I have to renew my work permit with a new passport. I sent a scan of the new passport to them via webform 1 week ago. I haven't heard back yet, not even an email confirming that they sent it to the agent.
I don't think it's a problem at all. Actually they ask you for it at the end of the process if your passport is expiring.
