How many Declaration Spouse should be sent coz every application (for principal application and spouse) has one?

a copy of his or her permanent residence card, Canadian passport or Canadian citizenship card;
a copy of the document proving your kinship to this family member (official family record book (livret de famille), etc.).
so, do you have relatives in Quebec ? In india this could mean a ration card which lists the names of the family members. You're in China right ? do you have something called the 'Hukou' ?
