yes u need to book an appointment by visit the relavent hospital or using website. I did the appointment through online(Asiri surgical hospital) and went on booked date. Here the link you can book it online. You can only do the medical with the approved hospitals which i guess they have two hospitals, one is the below

for appointment they will charge around 3500LKR and then on hospital you have to pay 4500LKR therefore totally around 8500

everthing is good sign dont worry you got your ppr very soon .

and i think you using old form and old process time 17 months
but your time lines little speedy so dont worry.
if it took more then month contact colombo visa ofice .
