Previous name on birth certificate

Dear all,

I have a question about the birth certificate. I understand that birth certificate is not mandatory and mine has the previous name I have used. Is the notarized birth certificate considered as a proof for other name? Should I specify my other name and upload the birth certificate or answer NO for that question? I am afraid that if I answer no to that question, I will be in trouble when I am requested to upload the birth certificate. That previous name was used while I was in elementary school which is almost 20 years ago.

And also, I don't have the original copy of my birth certificate. Just the notarized one.
