My wife and I are about to sumit our passaports after receiving PPR and they ask us to attach Annex A (form to fill with name, address and signature)
They do not specify if we need to submit one form each, or if it's just one form for both with only primary applicant information.
Has anyone filled out this form yet??? Please advise, help will be greatly appreciated.

I am not sure for what I have paid but I paid around CAD $2300 upfront for 3 members...Do you think RPRF would be included in that?

Yes if RPRF paid following is the breakup
Permanent Resident Application Fee 2*550=1100
Right Of Permanent Residence Fee (RPRF) 2*490=980
Permanent Resident Application Fee for Child 1*150=150
TOTAL 2230
